How to Apply:

Review MGA New Producer Contracting Requirements
1. Complete application and IRS W9
2. Include a copy of your producer license and E&O declarations page.
3. Fax to us at 708-552-4479

Once the application and license are received we will prepare a contract and contact you for an appointment to go over our products and sign the application.
